"You’ve hired someone to do the remodeling, but it is your responsibility to prepare your home for the contractors."

Not only that but prepare yourself. Remodeling your home is a huge stresser, prepare for the ups and downs of it. Do the little things like: make sure that you can do as much cleaning as you can everyday. Dust and debris flying throughout the house is a norm. Doing everything you can is a must to keep your sanity over the long haul!

Yes, you are absolutely correct. I would even go a step further to make sure everything that is written in the contractors agreement is up to code. Calling a City inspector to consult on what is required by the city can save you a lot of $$$.

On the subject of making sure that your contractor's insurance is up to date;

It coundn't hurt to contact your own policy holder for your homeowners insurance and let them know that your going to have contractor's at your home for the next 3 or 4 weeks. The cost is small compared to the ramifications. In these days of lawsuits and litigation, It's a small price to pay.
